Filling in this application form is the first step in applying to attend the summer school Computing Our Universe. If possible, use the Web application form. Otherwise, submit the information by email (summer@kea.astro.umass.edu), fax (413 545 4223), or mail (Astrophysics Summer School, 520 Lederle GRC, Department of Physics and Astronomy, University of Massachusetts, Amherst, MA 01003-4525).

In addition you need to arrange for two letters of recommendation to be sent by email from professional scientists acquainted with your work or studies.
